Most rehabilitation facilities are not equipped to handle the tremendous demands associated with brain and spinal cord injuries. At QLI, that’s all we do. We are also is the largest specialized provider of its kind in the country, with more than $70 million of award-winning facilities spread across a beautiful 60- acre campus. Each of our clients has extensive access to the unrivaled resources that only QLI can offer.
Colladay Center – The Colladay Center, sometimes referred to as the Program Center, is both QLI’s corporate and clinical headquarters. Located at the center of our 54-bed rehabilitation campus, this visually striking and welcoming building houses many of our formal therapy spaces as well as office and meeting space for many of QLI’s clinical experts. Formal therapy spaces include a physical and occupational therapy gym, speech language pathology and life skills training area, neuropsychological testing area, and assistive and adaptive technology assessment and training spaces. Also located in the Colladay Center are offices for our 24-hour nursing team, registered dietician, psychology and family counseling teams, service coordinators, admissions team, and other administrative departments. The Colladay Center is in operation 24-hours/day, 365 days/year under the watchful eyes our residential supervision staff.
Six Residential Houses – Clients participating in QLI’s Tri-Dimensional Rehabilitation® Program will be provided with a private room in one of six specially designed residential units on our rehabilitation campus. These units, designed to function like a typical American home, provide our clients the opportunity to practice and relearn the independent living skills and activities of daily living they will need in order to successfully transition home. Our specially trained residential staff members work closely with QLI’s clinical teams to provide a continuous level of necessary support to ensure our clients are making the most out of their rehabilitation experience. Each of QLI’s six houses contains a modern kitchen, living rooms, laundry room, shower room, staff office, and either eight or ten private residential rooms.

Lied Life Center
The second largest building on QLI’s rehabilitation campus is our dynamic Lied Life Center. This modern, multi-use building has been carefully designed to offer our clients and their families a wide variety of community-based therapy opportunities. With a fitness center full of cutting edge rehabilitation equipment, art studio, hobby/vocational training room, technology center, recreation area, multiple meeting spaces, our HUB coffee shop, and a large multi-use auditorium, this 16,000 square-foot center functions as a versatile rehab tool and an effective means of preparing our clients to return to their own communities.

Summit Skilled Nursing Program – QLI’s Summit program is completely one-of-a-kind. Nowhere else in the entire country will you find a vibrant, 24-hour care community that is so well designed to meet the specialized needs of a young adult population. Every detail of this 73-bed program has been thoughtfully adapted to meet not just the medical needs – but also the physical, social, recreational, and emotional needs – of younger adults living with brain injury, spinal cord injury, or other neuromuscular disabilities. Every Summit resident enjoys a private room (with private bathroom) that and offers a view that looks out on well-manicured green space. The recently remodeled and expanded Summit dining room and event center, called the Four Corners Café, is a also great spot for social and recreational opportunities on a daily basis. Summit residents also have daily access to all of the incredible amenities, activities and resources of the adjacent Durham Center (insert hyperlink to DC tour ) located just down the hall.
Lied Assisted Living Apartments – QLI’s Lied Assisted Living Apartments are designed to provide individuals with moderate cognitive and/or physical needs with the type of periodic, specialized support necessary for them to live as independently as possible. These 52 private apartments are offered as either studio or one-bedroom units. Each spacious apartment contains a private restroom and shower as well as a kitchenette with a sink, microwave, and refrigerator. Apartments are located on one of four wings, each having its own private living room area. A community space with soaring ceilings and a 20-foot tall fireplace connect the four wings and also functions as a dining room for breakfast, lunch, and dinner each day. Assisted Living residents also have daily access to all of the incredible amenities, activities and resources of the adjacent Durham Center (insert hyperlink to DC tour ) located just down the hall.
Durham Center – At the heart of QLI’s East (long-term care) Campus lies the stunning 34,000 square-foot Durham Center. This beautiful structure provides both Summit and Assisted Living residents with a wide variety of activities to enrich their lives. The Durham Center includes incredible amenities such as our auditorium and movie theatre, health club, swimming pool and spa, cyber lounge, hobby room, salon, sports bar, sun room, multiple lounges and meeting areas, and more. In addition to these world-class amenities, QLI’s Durham Center is also staffed with a robust team of experienced staff members who ensure our residents remain a part of the greater Omaha community. QLI’s Durham Center staff are constantly inviting external entertainers, musicians, political figures, and others to spend time with QLI’s residents. QLI’s Durham Center staff also take more than 3,000 trips per year into the community, ensuring that our Summit and Assisted Living residents remain a part of the surrounding community by regularly going shopping, to movies, sporting events, concerts, vocational or volunteer opportunities, and much more.
Chamelion Club, Pool, Technology Room, Theater
QLI owns and operates two freestanding 15-bed apartment buildings in the Omaha community called Beacon Heights and Richland. These apartments are located near 90th and Maple Streets in Omaha. Applicants to either of these apartments must be meet the HUD requirements. QLI operates a specialized home health agency that is available as necessary for apartment residents.
